On passing the Online Screening Test and Interview, we will start the application/documentation process.
 

As you are aware the first step towards reaching Australia is to get you an approval from the Board of Nursing in Australia. For this you need to submit certain correctly filled documents as quickly as possible.

 

The DOCUMENTS are as follows:

 

  1. The Attached AHPRA Application Form.
  2. A DD in the favor of "Australian Health Practitioner Regulation Agency" for $450( Australian Dollar ).
  3. A copy of your birth certificate . The copy must be Notarized*. Birth certificate issued  only by the court or Panchayat is accepted.
  4.  A copy of your marriage certificate. The copy must be Notarized*. Marriage certificate issued  only by the court or Panchayat is accepted.
  5. A copy of your degree. The copy must be Notarized*
  6. A copy of  all your educational and additional certificates. The copy must be Notarized*
  7. A copy of your transcript. The copy must be Notarized*.
  8. A copy of all your Registration(‘s). The copy must be Notarized*
  9. Work Statement / Reference -The Referee must mention that you have been working with the hospital as "a fulltime Registered nurse"
  10. Authority letter
  11. First and last page of your passport. The copy must be Notarized*.
  12. 4 Passport size photographs.
  13. IELTS - The copy must be Notarized*.
  14. PCC  -  Issued by the  Passport office.
  15. C.V (In format)
  16. Proof of Identity -  Also please submit a certified copy for any of the proof from Category B and Category D

 

 

IMPORTANT INSTRUCTIONS:

 

1.* NOTARY - When a document is notarized, every page of each document must have the following statement recorded “This is a true copy of the original, which I have seen.”

The person doing the Notary must record the following

a) Signature

b) Print or stamp his/her name; and indicate the qualification for certifying e.g. Solicitor/Legal practionor, Magistrate or notary Public

c) Contact address and phone number, Registration No.

 

2. The name on all your forms should be the same as on your Birth Certificate.
